These Terms of Service ("Terms") govern your access to and use of the services provided by Affinity Podiatry ("Affinity Podiatry," "we," "us," or "our"), including but not limited to our website, mobile applications, and any related services (collectively, the "Services"). Affinity Podiatry provides podiatric services aimed at diagnosing, treating, and preventing foot and ankle conditions. Our services may include but are not limited to consultations, examinations, treatments, surgeries, and referrals to other healthcare professionals as deemed necessary by our Podiatrists.
2. Patient Responsibility
You understand and agree that it is your responsibility to provide accurate and complete information about your medical history, current health status, medications, allergies, and any other relevant information to our Podiatrists. You also acknowledge that failure to provide such information may impact the quality of care and treatment you receive.
3. Appointment Scheduling
Appointments with Affinity Podiatry may be scheduled by contacting us directly via phone or email. We strive to accommodate our patients' scheduling preferences; however, appointment availability may vary based on demand and practitioner availability.
4. Payment and Insurance
Payment for services rendered by Affinity Podiatry is due at the time of service unless alternative arrangements have been made in advance. We accept various forms of payment, including cash, credit/debit cards, and can provide receipts/invoices to be submitted to health insurance plans. It is your responsibility to verify coverage and understand any out-of-pocket expenses associated with our services.
5. Cancellation and No-Show Policy
We understand that circumstances may arise that require you to cancel or reschedule your appointment. We kindly ask for at least 48 hours' notice for appointment cancellations or changes. Failure to provide timely notice may result in a cancellation fee as outlined in our Cancellation Policy.

Affinity Podiatry is committed to protecting your privacy and confidential health information in accordance with applicable laws and regulations, including the Health Insurance Portability and Accountability Act (HIPAA).
